Job Title: Emergency Support Coordinator
Position Type: Full-time Contract from August 1, 2020 to March 1, 2021
Salary: Commensurate with Experience
Location: Mississauga (with travel in the GTA)
Deadline: July 30, 2020
Description: The successful candidate will help isolated, vulnerable persons/families living with disabilities (including intellectual disabilities and mental illness) gain knowledge and document plans to avert or reduce crises during the pandemic.
Duties and Responsibilities:
• Facilitate knowledge sharing/system navigation information in case of an emergency
• Facilitate dialogue within families to develop emergency care plans (ECP hereafter) with specified roles and responsibilities
• Support individuals with disabilities and families in documenting emergency care needs when accessing hospitals
• Mentor individuals with disabilities/caregivers to address financial, health and legal needs in case of illness or death during COVID-19
• Conduct virtual counselling sessions via video/phone
• Host virtual information sessions/legal clinics with lawyers and subject matter experts
• Research and disseminate resource information
• Document statistics/performance indicators
• Other duties as assigned
Qualifications:
• Post-secondary education in the social services field (BSW or MSW preferred)
• 3 to 5 years of case management experience, preferably with persons/families living with disabilities (including intellectual disabilities and mental illness)
• Sensitive to the needs of racialized/diverse/newcomer communities, particularly the Muslim community in the Greater Toronto Area
• Experience working from a framework that is trauma-informed, with a focus on racism, discrimination, Islamophobia and oppression, and ensuring a positive and safe space
• Knowledge of accessibility and passion for inclusion
• Strong documentation, facilitation and navigation skills
• Considerable experience using various software applications including MS Office Suite (Microsoft Word, Excel, PowerPoint etc.)
• Strong organizational skills and ability to work independently
• Cultural knowledge of disability amongst diverse communities including in South Asian, Middle Eastern and African households is an asset
• Communication skills in Arabic, Somali and/or Urdu is an asset
• Ability to meet with individuals virtually or through home visits
